Before downloading the printable application form, please note that it has some drawbacks. This organization is composed primarily of volunteers. A printed application can take extra weeks to process since the information has to be manually entered into the registration system and proofread. In some locations this means the course can fill up while the form awaits to be entered. In contrast, the online application is much quicker and easier to process because your private information is directly sent to our registration system.
